Please review the following insurance participation and referral requirements before submitting your appointment request.
Plans We Are Not In-Network With
We are not in-network with the following plans:
Medicaid (Amerigroup), Wellcare, CareSource, PeachCare, Ambetter (Private and State), Friday, Georgia Cigna Connect, BCBS Blue Value, and most Marketplace plans.
New Patient Insurance Restrictions
We are not currently accepting new patients with the following coverage:
Medicare, ChampVA, VA Benefits Case Policy plans, and Oscar.
Plans Requiring a Referral
A referral is required for patients with:
- TRICARE Prime
- Medicare
Insurance benefits and eligibility can vary by plan. If you are unsure whether we participate with your specific insurance plan or whether a referral is required, please contact our office before submitting your appointment request.
